Bill of RightsWe are committed to ensuring that all Vermonters have access to quality health care, regardless of their ability to pay. We accordingly offers to qualifying individuals free and discounted care in connection with medically necessary procedures performed at the Center. Eligibility for free or discounted care will be determined based upon the patient’s demonstrated financial need and without regard to the patient’s race, religion, sex, age, gender identity, sexual orientation or national origin.
ApplicationAn Advance Directive is a type of written or oral instruction which explains what health care is to be provided should you become unable to make your wishes known. It is the policy of Green Mountain Surgery Center to accept Advance Directives. However, based on organization conscience, we will not honor a Do Not Resuscitate (DNR) order. Should there be a medical emergency, appropriate medical care, including resuscitation, will be provided and you will be transferred to an acute care facility. If we have received a copy of your Advance Directive, a copy will accompany you to the receiving facility. More information on Advanced Care Planning can be found here.
